For clients receiving services under Colorado Medicaid (IHSS, Community First Choice, Personal Care, or Homemaking), your health information is protected under federal and state law.

• Access: You or your authorized representative have the right to inspect and receive copies of your records within 7 to 10 business days of a written request.

• Coordinate care with your Case Manager and physicians.
• Process payroll, taxes, and insurance for our caregivers.
• Improve our website and agency operations through our Quality Assurance & Performance Improvement (QAPI) program.
• Comply with mandatory reporting requirements for suspected abuse, neglect, or exploitation.

Disclosure of Information

We do not sell your personal information. We may share your information with:

• Authorized Payers: Such as Colorado Medicaid (HCPF) for billing and reimbursement.
• Regulatory Agencies: Such as the Colorado Department of Public Health & Environment (CDPHE) during surveys or audits.
• Legal Authorities: When required by law or to protect the safety of our clients.

Security of Your Data

We implement HIPAA-compliant cloud storage and encryption for all electronic records. Our staff is trained annually on HIPAA and data security protocols. Please be aware that while we use secure systems, no method of transmission over the internet is 100% secure.

Your Choices and Rights

• Opt-Out: You may opt out of receiving non-essential communications from us at any time.
• Record Corrections: You may request that we correct inaccurate information in your file.
• Grievances: If you believe your privacy rights have been violated, you may file a complaint with our Administrator or directly with the U.S. Department of Health & Human Services. We will not retaliate against you for filing a complaint.
